Scheduler – Home Health Care

At Henry Ford Home Health Care ,we are dedicated to providing high-quality home health services that support patients in managing complex medical needs outside of a hospital setting. Our team includes nurses, therapists, dietitians, and social workers ,all working together to provide exceptional care.
As a Referral and Scheduling Coordinator ,you will play a vital role in processing referrals , scheduling patient visits ,and acting as a liaison between clinicians and customers. General Summary - Responsible for processing referrals and scheduling customer visits for all Home Health Care clinical disciplines.
- Act as liaison between current customers and clinicians as it pertains to service issues.
- Provide office support and act as a resource for team members, medical staff, and other professionals.
Education/Experience Required - High school diploma or G.E.D. equivalent.
License/Certifications - CNA License.
- Completion of a Nurse Assistant program (titles for similar programs may vary, e.g., Patient Care Technician (PCT) OR
- Patient Assistant Technician (PAT) but perform the same job role.) OR May be currently enrolled in a nursing program and have successfully completed the Fundamentals of Nursing course. OR
- Foreign trained and licensed RN who has relocated to the United States and does not hold a Michigan Rn license. OR
- Medical Assistant program with OR without experience. OR
- At least one (1) year of previous experience in an NA, PCT, PAT, or similar job role.
Requirements - Complete BLS Heart Saver Certification during orientation.
