Job Title: Physical Therapy Assistant – In-Home Rehab Specialist

Company: Home Health Care, Inc.

Schedule: Full-Time or Part-Time with weekend rotation

Compensation: $42 - $46 per visit + mileage reimbursement

What You’ll Do:

  • Carry out rehab plans under a licensed PT’s guidance for homebound patients
  • Work directly with clients on physical improvement goals using progressive, evidence-based techniques
  • Deliver functional training to promote fall reduction and restore independence
  • Document treatment sessions and patient responses for team insight and ongoing care adjustment
  • Engage with interdisciplinary team members to ensure wraparound support for every client

Qualifications:

  • Current Physical Therapy Assistant license in Minnesota
  • Access to a vehicle, valid license, and insurance
  • Passion for in-home care and commitment to client progress
  • Open to experienced professionals and newer grads seeking meaningful roles
